Download Instructions (Win 95)

1. To save: right click the link below and select "Save as Target" (using Internet Explorer) or "Save as" (using Netscape) on the drop down dialog box. Then choose the location to save file for later viewing or printing.

2. Printing Tip: On the print dialog box make sure the "Shrink to Fit" box is checked.

3. If you have the Acrobat Reader Plug-in installed on your computer click on the link to launch the file in your browser.
